Creekland Middle School students are encouraged to become involved in the many activities provided for enrichment and enjoyment. Joining a team or club will help you make new friends and give you a sense of accomplishment.

We also encourage students to consider trying out for the school basketball or cheer-leading teams. Everyone should come out and support their friends and school during the season.
Girls Basketball:
All Grades - Monday, October 2nd
6th & 7th Grade - Tuesday, October 3rd
7th & 8th Grade - Wednesday, October 4th
Thursday & Friday, October 5-6 - call backs only
All forms are available and must be completed and uploaded on Rank One for student participation. Go to www.rankone.com and create account and upload the four required forms.
Required forms include the following:
1. Signed CCSD Athletic Information form (requires student signature to complete)
2. Insurance Information (see link on site for further direction if no insurance currently)
3. Signed CCSD Signature Page form (requires student signature to complete)
4. Physical form - All four pages of this form must be completed and uploaded
STUDENT WILL NOT BE ALLOWED TO TRYOUT WITH OUT FULL ATHLETE PARTICIPATION PACKET FULLY COMPLETED

Cheer - September 5th
Boys BB - September 23rd
Girls BB - September 30th ***It is highly recommended that these forms are submitted at least 48 hours prior to deadline to allow time for review and feedback in case required information is missing or upload did not work properly so student isn't excluded from tryout***
Home Study Participation – Parents/Guardians of students interested in part-time enrollment for participation in grades 6-12 extracurricular activities shall submit the following documents to the Office of School Operations via email to Tonya.Sebring@cherokeek12.net:
A copy of the student’s most recent annual progress assessment report. Additionally, a transcript from the home study program is required if participating in a GHSA sport or activity. The transcript shall include the number of cumulative credits earned to date.
A copy of the State of Georgia Home Study Program Declaration of Intent Form filed with the Georgia Department of Education.
